Main Responsibilities
- Provide and coordinate patient care, and educate patients and families about their health conditions.
- Assess patients, identify their needs, and carry out and monitor their care plan.
- Work with physicians and allied health teams in different care settings.
- Collect patient data through regular assessment and re-assessment.
- Define nursing diagnoses and expected outcomes, and review care plans with the team.
- Record vital signs and quickly escalate any abnormal findings.
